You may have recently received information from the Centers for Medicare & Medicaid Services (CMS) about the Medicare Health Outcomes Survey (HOS) or Medicare Health Outcomes Survey-Modified (HOS-M). If you received the HOS or HOS-M questionnaire in the mail, we hope you’ll complete and return it. If you are not able to return the questionnaire, one of the survey organizations working with CMS may contact you by telephone. In 2024, four survey organizations are working with CMS on the survey: CSS, DataStat, Press Ganey, and Qualtrics.

Your voice matters.

Your responses will be used to help CMS improve the quality of health plans and help others choose a health plan. The survey takes just a few minutes to complete, and your information is kept private by Law. Participation is voluntary.

Your privacy is very important to CMS.

All information you provide will be held in strict confidence and is protected by the Privacy Act. Your information will be used only for the purposes permitted by law and it will not be disclosed or released for any other reason. We will not share your name, contact information, or your responses with any medical providers or salespeople.

What to do if you have questions or concerns about filling out the survey.

You can also use the information below to connect with the Customer Support line of the survey organization contacting you. To identify your survey organization, refer to the mailing address printed on the last page of the questionnaire you received and match it with the “Survey Processing Address” below.
